Wellness Center Member Agreement

 

The following are the guidelines and policies of membership at the SouthCrest Wellness and Sports Medicine Center:

 

Parking

 

There are two areas designated for wellness center parking:

 

·         The East parking area faces Mingo Road from which you may enter through the double doors that have the Wellness Center logo.

·         The West parking area is directly in front of the ticket booths and the football field and is used as primary parking for UMAC events. Our entrance from the West parking area is also a set of double doors that have the Wellness Center logo located just north of the U-Wear store.

·         Please do not park in the loading dock, plaza, driving lanes, on medians or any unauthorized area.

 

Dress Code

 

·         The entire foot has to be covered at all times while in the center, no sandals, Tevas, etc.

·         Shirts must be worn at all times. No profanity, inappropriate or offensive statements, gestures or pictures allowed.*

·         Jeans, cutoffs or clothing with rivets, spikes, etc, are not allowed. Pants must be worn at or above waist level.

 

Code of Conduct

 

·         Please limit cardio equipment use to 30 minutes while others are waiting.

·         All persons must be at least 14 years of age to attend classes or exercise in facility.

·         Children 13 or younger cannot be left unattended in the Wellness Center or UMAC while parents exercise or attend classes.

·         Chalk is not allowed in the center.

·         Profanity of any type is not allowed anywhere in the center.

·         Please do not drop the weights on the floor or place them on the upholstery of the benches.

·         Please do not “Park” on the weight machines, if someone is waiting to use the machine you are on please allow them to work-in with you.

·         Please wipe down your machine after use, if you sweat excessively ask a personal trainer or front desk staff for a cleaning towel.

·         Please remove and re-rack your weights if you are finished using them.

·         No food or drink allowed on the wellness center floor except water in a covered container.

·         Proper personal hygiene is required.

·         Please do not slam the weight stacks.

·         When using tredmill please do not attempt to exit belt until it has come to a complete stop.

 

Personal Training

 

The Personal Training program provides you with one-on-one contact and guidance with a certified personal trainer. 15-minutes sessions are available by appointment at $10.00 each.  

 

You as a new member will be offered five free fifteen minute sessions** that can get you started and headed in the right direction to meet your fitness goals. These sessions are optional and can also be a great opportunity to have your current fitness program evaluated.

I have read, understand, and agree to abide by all policies and guidelines as a member of SouthCrest Wellness and Sports Medicine Center, and further understand that non-adherence or compliance could result in an interruption of my membership privileges.

**Initial appointment must be set within one week of joining and all sessions must be completed during first 90 days of membership.
