Wellness Center Enrollment Information
The enrollment packet for the Wellness Center consists of four forms: Informed Consent, Member Agreement, Enrollment Form and Financial Forms. We have posted three of the forms below for you to print off, fill out and take with you when you go to the Wellness Center to enroll. The financial forms will need to be filled out at the time of your actual enrollment.
