When you arrive at SouthCrest Surgicenter for your scheduled surgery, you will be greeted and registered by our receptionist. 

You will be asked for your insurance card(s), and to sign a release of information form so that we can file your health insurance. Our admitting nurses will then be informed that you have arrived.
