Birth Certificate Information
Once your baby is born, the birth certificate clerk will bring a “Birth Certificate Info Sheet” to your room for you and/or your spouse or the father of baby to complete. Any questions should be referred to the birth certificate clerk at 294-4292. The birth certificate clerk will return in about an hour to collect the completed information sheet and to review all the information with you regarding accuracy, spelling and completeness.
The birth certificate clerk will then go to the Nursery to retrieve any additional statistics needed concerning the newborn, such as: time of birth, weight, length or Apgar score. The birth certificate clerk will use this information to complete the information sheet from the Social Security Office.
After completing this process, the clerk will return to your room to have you review all the information one last time. She will also explain how to get copies of the birth certificates, give you information on Social Security, and have you sign the completed certificate.
